Nurse Practitioner

Virtual Role | Value-Based Care Organization

Location: New Mexico (Remote, with periodic onsite all-staff meetings)

Reports To: Clinical Team Manager or SVP, Clinical Operations

Schedule: Monday–Friday, 8:00 AM–5:00 PM, with rotating on-call

About the Role

We are an innovative healthcare organization expanding our physician-led, care-at-home model for medically complex Medicaid and Dual Eligible members. We’re seeking a Nurse Practitioner to join a virtual interdisciplinary team and help launch our New Mexico clinical program.

This is not a traditional clinic role. You’ll combine direct patient care, care coordination, and program development, working alongside physicians, RNs, social workers, occupational therapists, health plan case managers, and community-based caregivers to proactively manage patient health, reduce unnecessary hospital utilization, and improve quality outcomes.

What You’ll Do

• Deliver high-quality virtual clinical care for medically complex patients, including assessments, medication management, care planning, and telehealth follow-up
• Coordinate care with primary care providers, specialists, managed care organizations, and community partners
• Guide patients and caregivers through care transitions, including hospital admission, discharge, and hospital-to-home planning
• Participate in a rotating 24/7 triage and after-hours support model to enable early intervention and reduce avoidable ED visits and hospitalizations
• Monitor patient populations using technology-enabled care management platforms, identifying opportunities for proactive intervention
• Contribute to the development of clinical protocols, workflows, and operational processes as the program grows
• Support and mentor teammates, helping build a culture of clinical excellence and innovation
• Maintain compliance with applicable state and federal regulations and organizational clinical standards
• Participate in regular 1:1s and team meetings to stay aligned with organizational and clinical goals

What We’re Looking For

Required

• Current Nurse Practitioner license in New Mexico
• Family Health or Adult/Gerontology National NP Certification
• DEA registration with full prescriptive authority
• 3–5+ years of NP experience, with the ability to exercise sound clinical judgment and manage patient care with a high degree of autonomy
• Significant experience with telehealth, virtual care delivery, or remote patient management
• Experience with value-based primary care, population health, managed Medicaid, or accountable care models, including HCC documentation and quality/care gap closure
• Strong communication, organizational, and interdisciplinary collaboration skills

Preferred

• Experience serving elderly, disabled, TBI, IDD, or other medically complex populations
• Background in rehabilitation medicine, long term care, emergency medicine, care transitions, home-based care, or utilization management
• Multi-state compact RN license
• Active Medicare and Medicaid provider number
• Entrepreneurial mindset, comfortable in a fast-paced, startup-like environment

Work Environment

• Primarily remote, with a technology-enabled care model built on advanced EMR and population health management tools. On-site meetings occur periodically for all-staff alignment. You’ll join a fast-paced, well-supported startup environment with the opportunity to shape New Mexico’s clinical program from the ground up.
