Base Salary: $125,000 – $145,000
+ Performance-Based Bonuses

Position Overview

We are seeking a licensed Nurse Practitioner (NP) in the state of Minnesota to join our growing remote care team. This role is ideal for a provider experienced in telehealth visits, chronic care management, and digital health platforms.

As a Nurse Practitioner, you will conduct virtual patient visits, support RPM program enrollment, and provide ongoing clinical oversight for patients managing chronic conditions.

Key Responsibilities

• Conduct telehealth visits (video/phone) with patients
• Perform initial patient establishment visits, including:
• Obtaining consent for RPM services
• Reviewing medical history and medications
• Develop and manage care plans for chronic conditions (e.g., hypertension, diabetes, CHF)
• Monitor and respond to RPM alerts and patient data trends
• Collaborate with:
• Pharmacists
• Supervising physicians
• RPM care team
• Document all encounters accurately in EHR systems
• Provide patient education on:
• Device usage
• Disease management
• Medication adherence

Required Qualifications

• Active, unrestricted Nurse Practitioner license in Minnesota
• National board certification (FNP, AGNP, or equivalent)
• Current DEA license (preferred but not required)
• Active NPI number
• Ability to work under a collaborative/supervising physician model (as required by state law)
• Minimum 1–2 years of clinical experience

Preferred Experience

• Experience with telehealth / virtual care delivery
• Familiarity with EHR platforms (e.g., CureMD or similar systems)
• Experience with Remote Patient Monitoring (RPM) or Chronic Care Management (CCM)
• Strong comfort with technology and remote patient engagement tools
