HealthCare Support is seeking a Remote Nurse Practitioner or Physician Assistant to join a rapidly growing healthcare provider serving patients in Airzona. (REMOTE – You can live anywhere in the state of AZ as long as you are licensed in AZ.)
This is a unique opportunity to deliver high-quality, patient-centered care directly in the home—focusing on Chronic Kidney Disease and primary care needs. If you’re passionate about building relationships, improving outcomes, and working with autonomy while still being part of a collaborative team, this role is for you.
Schedule: Monday – Friday, 8:00 AM – 5:00 PM – REMOTE
Compensation Notes: $125,000 – $130,000 base salary + 10% annual bonus
Daily Responsibilities for the Remote Nurse Practitioner / Physician Assistant:
- Develop and implement individualized treatment plans
- Order and interpret diagnostic tests and lab work
- Prescribe medications as appropriate
- Provide ongoing patient education and chronic disease management
- Complete accurate and timely documentation of services
- Collaborate with interdisciplinary care teams and participate in routine meetings
- Utilize telehealth to support patient care

Required Qualifications for the Remote Nurse Practitioner / Physician Assistant:
- Active Nurse Practitioner (NP) or Physician Assistant license in Arizona
- 2+ years of experience as a practicing NP or PA
