Remote Nurse Practitioner: Remote in Pennsylvania

Compensation: Starting at $65 per hour

Position Overview:
We are seeking a licensed Nurse Practitioner (NP) to perform initiating visits for care management programs, including Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI). This is a contract-based, telehealth position focused on conducting virtual face-to-face visits with patients to establish and initiate care management services.

Key Responsibilities:

• Conduct initiating visits with enrolled patients via telehealth (video visits)
• Establish care plans and document appropriately for care management services
• Complete visits efficiently, with the ability to see multiple patients per hour
• Ensure all documentation supports billing and compliance requirements
• Collaborate with supervising physicians when clinical questions arise
• Maintain accurate and timely charting within the provided systems

Patient Volume Expectations:

• Initial enrollment volume of approximately 700 patients
• Ongoing visit demand based on program growth and patient needs

Requirements:

• Active Nurse Practitioner license in the state of Pennsylvania
• Must already be credentialed with payers (required)
• Active NPI number
• Must carry individual professional liability and malpractice insurance
• Experience with telehealth and/or care management services preferred
• Strong clinical documentation and time management skills

Work Structure:

• Flexible scheduling based on availability and patient demand

• Access to supervising physicians for clinical guidance and escalation when needed
