TeleMD Health is hiring nurse practitioners — full-time and per diem (PRN) — to deliver remote Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) to patients across New York, Florida, Arizona, and Texas. This is 100% remote work with steady patient panels, flexible scheduling, and a supportive clinical team behind you.

We’re filling our core full-time roles first, then building a per-diem bench. Multi-state NPs are strongly encouraged to apply — if you hold licenses in several of our states, we want to talk.

COMPENSATION: $75–85/hr (1099) + performance bonus

SCHEDULE: Full-time or per diem — your choice

GROWTH: This is a 1099 contract role built to grow. After an initial 90-day period, malpractice and health coverage can be added as part of your bonus package as you build tenure and your patient panel.

WHAT YOU’LL DO

– Manage a panel of CCM/RPM patients via telehealth

– Conduct monthly care check-ins and keep care plans current

– Document in our EMR and coordinate with the care team

WHAT YOU NEED

– Active, unencumbered NP/APRN license in one or more of: NY, FL, AZ, TX

(additional licenses in WA, NV, OR, GA, MA, NM, CO, or KS a strong plus)

– Current NPI

– Comfortable with telehealth visits and EMR documentation

– Reliable high-speed internet and a private workspace
