Job title: Telehealth Triage Registered Nurse (RN)
Purpose/Summary: The Telehealth Triage RN serves as the first-line clinical support nurse for the After-Hours Telehealth (AHTH) program and assists with the Remote Patient Monitoring program. This role is responsible for triaging inbound calls, reviewing clinical handoffs from skilled nursing facilities (SNFs), assessing patient acuity, prioritizing follow-up activities, and facilitating timely provider intervention when necessary. The Telehealth Triage RN utilizes critical thinking and clinical judgment to identify residents at risk for deterioration, proactively communicate with facility staff, and ensure urgent clinical concerns are escalated appropriately. This position will report to an AHTH Clinical Manager.
This position is remote and works from home. The hours will vary but include the range of after 5 pm Est until 3 am EST Monday – Friday, day and night shift coverage on weekends and will include some holiday coverage.
This position works closely with telehealth Nurse Practitioners and Physician Assistants to improve patient outcomes, reduce avoidable hospital transfers, and support high-quality after-hours clinical care.
Role & Core Responsibilities:
Clinical Handoff Review & Prioritization
- Review incoming facility clinical handoffs at the beginning of each shift.
- Triage inbound calls from facilities utilized clinical protocols and clinical acumen
- Assess resident conditions using clinical judgment to determine urgency and required follow-up.
- Prioritize residents based on risk factors, clinical status, and likelihood of deterioration.
- Identify residents requiring immediate provider evaluation versus routine follow-up.
Facility Outreach & Triage
- Contact nursing staff at SNFs to obtain clinical updates on residents identified during handoff review.
- Perform nursing triage and assess reported symptoms, vital signs, clinical changes, and nursing concerns.
- Determine when issues can be managed through nursing guidance versus provider involvement.
- Escalate urgent or concerning findings to the telehealth provider for evaluation.
- Facilitate warm handoffs between facility staff and telehealth providers when provider intervention is indicated.
- Document all triage activities according to company standards.
Proactive Clinical Surveillance
- Conduct proactive outreach calls to facilities regarding high-risk residents as directed by leadership or identified through handoff review.
- Monitor residents with recent emergency department visits, hospital discharges, significant clinical changes, abnormal laboratory findings, or other identified risk factors.
- Collaborate with providers to ensure continuity of care and timely follow-up.
Remote Patient Monitoring (RPM)
- Review Remote Patient Monitoring (RPM) dashboards and clinical alerts.
- Identify trends suggesting clinical decline or worsening chronic disease management.
- Escalate abnormal findings, concerning trends, or high-risk alerts to telehealth providers for further evaluation.
- Assist in developing workflows that support effective RPM utilization and early intervention.
Provider Support
- Serve as a clinical resource for telehealth providers by gathering pertinent clinical information prior to evaluation.
- Assist with communication between facilities and providers.
- Support efficient workflow management during periods of high call volume.
- Help ensure urgent patient concerns are addressed in a timely manner.
Quality & Compliance
- Maintain compliance with all company policies, nursing standards, state regulations, HIPAA requirements, and facility agreements.
- Participate in quality improvement initiatives aimed at reducing avoidable hospitalizations and improving patient outcomes.
- Maintain accurate and timely documentation of all clinical interactions.

Qualifications:
Required
- Active unrestricted Compact Registered Nurse (RN) license. Additional state-specific licensure may be requested as well.
- Minimum 3 years of clinical nursing experience.
- Experience in Emergency Nursing and Telehealth.
- Strong assessment, triage, and clinical decision-making skills.
- Excellent verbal communication and customer service skills.
- Ability to work independently in a fast-paced remote environment.
- Proficiency with electronic health records and telehealth technology.
Preferred
- Experience in Skilled Nursing Facility / Long term Care settings.
- Experience with telehealth triage.
- Experience with Remote Patient Monitoring (RPM) programs.
- Knowledge of common geriatric and chronic disease conditions.
- Experience working with interdisciplinary care teams.
- Single state RN licensure in states not currently in the Compact.
Working conditions:
A designated, secure, and private home workspace is required for this remote role to ensure compliance with HIPAA and company privacy and security policies. This workspace must support confidential patient communications and prevent unauthorized access to protected health information. Typing is required, and you will be required to use Company approved virtual technology to visualize patients during visits. A personally owned computer used for this role must meet all company-mandated security and encryption requirements as defined by the IT team. Company-provided security software, including platforms for patient video consultations, may need to be installed to ensure the device remains compliant with organizational data protection standards. Periods of prolonged sitting or standing can be expected. Travel requirements could occur based on specific state licensure and leadership needs.
Compensation: $38-$45/hr. Compensation is determined based on market range, experience, skills, education, licensure and qualifications.
