Virtual Family Nurse Practitioner (Telehealth) – Family Practice
Position Summary-
We are seeking a Board-Certified Family Nurse Practitioner to provide comprehensive virtual primary care services to patients throughout California. This is a fully remote telehealth role in which you will partner closely with an in-home clinical care team to deliver high-quality preventive and primary care. The Nurse Practitioner conducts all patient visits virtually while collaborating with field clinicians who perform hands-on assessments and diagnostic services during the visit.
Candidates may reside in one of several approved states but must maintain a work schedule aligned with Pacific Time (PST).
Key Responsibilities
- Conduct comprehensive telehealth visits with patients in a virtual family practice setting.
- Review daily patient schedules, prepare technology, and ensure all digital tools are ready before appointments.
- Utilize modern telehealth platforms to manage appointments, documentation, and communication with the interdisciplinary care team.
- Build rapport with patients while obtaining comprehensive medical histories and assessing current health concerns.
- Perform health risk assessments, medication reconciliation, cognitive screenings, and preventive health evaluations.
- Provide patient education focused on chronic disease management, preventive care, and healthy lifestyle practices.
- Collaborate in real time with in-home clinical team members to review vital signs, coordinate laboratory testing, and address immediate patient care needs.
- Develop and implement individualized care plans based on clinical findings.
- Manage preventive care interventions and appropriate screenings.
- Coordinate referrals to specialists and community resources when indicated.
- Review and manage medication therapy, including adjustments within scope of practice.
- Maintain accurate, timely, and compliant electronic medical documentation.
- Participate in interdisciplinary collaboration to support continuity of care and improved patient outcomes.
Required Qualifications
- Board Certified Family Nurse Practitioner (FNP)
- Active California Family Nurse Practitioner license
- Credentialed Medi-Cal (California Medicaid) provider
- Minimum of 1.5 years of clinical experience in Family Practice, Primary Care, or Internal Medicine
- Excellent virtual communication and patient engagement skills
- Strong clinical assessment and documentation abilities
- Comfortable utilizing telehealth platforms and electronic medical records
Preferred Qualifications
- Several years of telehealth clinical experience
- Bilingual in Spanish and English
- Experience caring for Medicaid populations
- Experience in value-based care, preventive medicine, or population health
Work Schedule
- Monday through Friday
- Approximately 7:00/7:30 AM – 3:30 PM Pacific Time
- Must be available to work Pacific Time hours
