Position Summary

Holon Health is seeking an experienced and compassionate Nurse Practitioner licensed in Illinois to support individuals with SUD through collaborative, integrated care. In this role, you’ll work closely with Care Navigators, Recovery Coaches, Case Managers, judicial partners, and community-based organizations to deliver care that addresses the full scope of a client’s mental, emotional, and physical needs.

This position offers remote flexibility and can be structured as either full-time (40 hours/week) or part-time (20 hours/week) depending on your availability and interest. You’ll provide telehealth-based assessments, medication management, and care coordination to support clients on their recovery journey—and be part of a forward-thinking team breaking down barriers to behavioral healthcare.
