The Remote Advanced Practice Provider (APP) at ChenMed, part of ChenMed, is a vital member of a rapidly expanding telehealth team, delivering innovative, virtual care to patients across the United States. This role serves as the dispositional authority for after-hours and weekend clinical calls, providing expert diagnostic, treatment, and documentation of patients’ health status. The APP collaborates with physicians and multidisciplinary teams to develop and implement patient management plans, ensuring high-quality outcomes through telemedicine. Compensation ranges from $111,140 to $158,771, with eligibility for bonuses or commissions. Responsibilities:
  • Responsible for assessing acute and non-acute clinical problems through virtual video conference or telephone consultations.
  • Take ownership of performing and documenting physical evaluations and patient histories, analyzing trends and developing patient management plans.
  • Aid in the development of care plans, including health education, referrals, and patient/family counseling.
  • Collaborate cross-functionally to ensure compliance with guidelines and report health data with the patient’s PCP and other providers.
  • Deliver high-quality patient care by utilizing telemedicine to achieve optimal outcomes and participate in quality improvement activities.
  • Lead efforts to communicate and collaborate with physicians, patients, and team members using various formats and technologies.
  • Help drive innovation in telehealth practices by utilizing data to demonstrate program effectiveness and success.
  • Play a key role in shaping telehealth policies, procedures, and protocols in partnership with the clinical team.
  • Provide extraordinary customer service and professionalism to all internal and external customers.
Education:
  • A Bachelor’s degree in Nursing (BSN) and graduation from a school of nursing for Advanced Practice Nursing with certification in area of specialty are required.
  • A Master’s degree in Nursing is required.
  • Board certification by AANP or ANCC is required.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required upon hire.
  • Compact multistate Nursing licenses are required.
  • Multi-state APRN licensure to include Florida and at least two licenses in the following states: VA, GA, MI, MO, OH, PA, TN, TX, IL, KY, LA are required.
Skills:
  • Demonstrated record of consistently achieving clinical performance metrics is required.
  • Expert-level business acuity and knowledge of medical practices are essential.
  • Technical capability to conduct telemedicine visits in accordance with state and federal regulations is required.
  • Ability to demonstrate excellent clinical judgement, problem solve, and prioritize under pressure is necessary.
  • Proficiency in Microsoft Office Suite products including Excel, Word, PowerPoint, and Outlook is required.
  • Ability and willingness to obtain independent/autonomous practice as an NP in applicable states is required.
  • A minimum of three years’ acute/primary/urgent care clinical work experience as a Nurse Practitioner is required.
  • At least two years’ telehealth work experience is preferred.
  • Spoken and written fluency in English is required; bilingual (Spanish/Creole) is a plus.
