Remote Behavioral Health Quality Management Specialist (RN) – IL, TX, NM

Position:- Medical Health Management Specialist

Rate:- $45/hr.

Working hour :-40 hours per week
Location:- Hybrid in Chicago, IL (North and West sides), (mostly the northside, but there could be the occasional odd case in the south/west side.)

Waiver position – Note :- Required Job Requirement

• License required LPC, LCSW, LCPCs and RNs
• 1+ year Quality improvement experience.
• 3+ year Clinical experience

Job Overview:
We are seeking a skilled and compassionate professional to support medical management and health education initiatives for members enrolled in government healthcare programs. This role focuses on improving patient outcomes through clinical guidance, data-driven insights, and collaboration with internal and external stakeholders.

Key Responsibilities:

• Conduct medical management and health education programs for members
• Analyze and interpret clinical and regulatory data to support reporting requirements
• Serve as a primary point of contact for members, addressing medical needs and care coordination
• Collaborate with physicians, care teams, and external vendors to ensure effective service delivery
• Provide clinical insights and recommendations to improve care quality and outcomes
• Support wellness initiatives and managed care programs

Required Qualifications:

• Active, unrestricted license in one of the following:
• Registered Nurse (RN) with 3+ years of direct clinical experience
• Licensed Professional Counselor (LPC)
• Licensed Master Social Worker (LMSW) with required clinical experience
• Minimum 3 years of experience in wellness or managed care, including interaction with members and physicians
• Strong understanding of healthcare systems, wellness programs, and industry trends
• Excellent verbal and written communication skills
• Experience in medical data analysis and reporting
• Proficiency in MS Office (Word, Excel, PowerPoint) and web-based applications
• Valid driver’s license, reliable transportation, and willingness to travel within the assigned territory

Preferred Qualifications:

• Clinical experience in patient education or condition management
• Transition of Care experience
• Experience managing complex or catastrophic cases
• Bilingual (English/Spanish)
• Certification in Case Management, Training, Project Management, or related healthcare fields
