Nurse Practitioner — Clinical Lead

Type: Contract-to-permanent

Location: New York (licensed); multistate preferred

Compensation: $120K–$140K annualized

Role summary

We are building a telehealth practice focused on endocrine and metabolic health, with a core emphasis on weight management and diabetes care. We are hiring our first clinician to deliver patient care, develop clinical protocols, and shape the care model from the ground up. This is a clinical leadership role, not a shift-filling position — your judgment and input will directly define how the practice operates and scales.

Key responsibilities

  • Deliver telehealth-based care across a structured visit protocol (synchronous and asynchronous) within an endocrine and metabolic health program focused on weight management and diabetes
  • Develop and refine clinical protocols for medication titration, glycemic management, side-effect triage, dose escalation, and treatment transitions
  • Design clinical workflows in collaboration with the technology and operations team: patient intake, visit structure, documentation, prescribing, and follow-up
  • Collaborate with an endocrinologist advisor on complex cases and protocol refinement
  • Contribute clinical content to structured reports delivered to referring physicians
  • Document clinical decision logic, escalation criteria, and care pathways to support future systematization
  • Participate in weekly operational reviews, surfacing clinical workflow friction and patient experience insights

Qualifications

  • Active NP license in New York State (FNP, AGNP, or ACNP)
  • Multistate licensure strongly preferred — NLC compact enrollment covering additional states is a meaningful advantage
  • Hands-on experience managing patients with diabetes, obesity, or metabolic conditions, including prescribing and titrating GLP-1 therapies, insulin, and related medications
  • Background in adult or geriatric care settings — internal medicine, primary care, or a related discipline where complex chronic disease management is central to the work
  • Current payer credentialing in New York, or an active CAQH profile enabling rapid enrollment
  • Comfort developing clinical protocols — writing the playbook, not just following one
  • Strong collaboration skills across a multidisciplinary team (engineering, operations, and business)

Preferred

  • Experience delivering care in a telehealth or virtual setting
  • Board certification in obesity medicine, diabetes care and education, or endocrinology-adjacent specialty (not required, but relevant)
  • Experience with Athena EHR
  • Familiarity with remote patient monitoring workflows
  • Experience working alongside dietitians, diabetes educators, or nutrition counselors as part of a care team
  • Leadership, training, or team development experience
