Role Description This is a per diem, SOC RN role supporting patients who receive home health services in San Joaquin County. The Registered Nurse will conduct in person assessments, develop and update individualized care plans, and coordinate with the interdisciplnary team to ensure continuity of care. Daily responsibilities include monitoring patient progress, reviewing clinical documentation, providing patient and family education, and offering clinical guidance to the LVN and CHHA field staff. The role may include triaging patient concerns, reviewing lab results and medications, and participating in quality improvement activities to promote safe and effective home care.
Qualifications
- Active, unrestricted Registered Nurse (RN) license in the relevant state(s); BSN preferred.
- Experience in home health, community health, medical-surgical, or related clinical settings.
- Strong assessment, critical thinking, and care-planning skills, including the ability to prioritize multiple patients’ needs.
- Proficiency with electronic health records.
- Excellent written and verbal communication skills for educating patients, families, and caregivers.
- Demonstrated ability to work independently in a remote environment while collaborating effectively with an interdisciplinary team.
- Knowledge of home health regulations, documentation standards, and evidence-based nursing practices (OASIS experience is a plus).
- Current BLS (Basic Life Support) certification; additional relevant certifications (e.g., home health, gerontology, chronic disease management) are beneficial
