About Dizney Behavioral Health

Dizney Behavioral Health is a small, independently owned outpatient mental health and addiction medicine practice located in Berlin, Connecticut.

We are seeking a compassionate, motivated Nurse Practitioner (FNP or PMHNP) to join our growing team on a per diem or part-time basis, with the potential to increase hours and eventually transition into a full-time position as the practice expands.

Our practice currently consists of one Nurse Practitioner and an administrative assistant. We pride ourselves on providing individualized, patient-centered care in a supportive, collaborative environment.

We recognize that every provider brings different experiences, strengths, and areas of clinical interest. Our goal is to create a flexible work environment that promotes clinical autonomy, collaboration, and professional growth.

Position Overview

  • Flexible per diem or part-time scheduling, with the potential to increase hours.
  • 100% telehealth available, with physical office space in Berlin, CT for providers who prefer in-person appointments.
  • W-2 employee position.
  • No routine on-call responsibilities.
  • Occasional coverage for the other provider during planned vacations, illness, or unexpected absences, primarily involving medication refill requests and continuity of care.
  • Administrative support provided.

  • Substance use disorders and addiction-related concerns
  • Other common outpatient psychiatric conditions

Experience in addiction medicine is welcomed but not required.

We understand that not every provider is comfortable treating every diagnosis or prescribing every medication. We are happy to collaborate and work within each provider’s clinical experience, training, and comfort level when determining patient assignments.

Qualifications

  • Active Connecticut APRN license.
  • FNPs and PMHNPs are both encouraged to apply. PMHNP certification is NOT required.
  • Experience in outpatient psychiatry, mental health medication management, or addiction medicine preferred.
  • Appropriate Connecticut prescriptive authority.
  • Strong clinical judgment, communication skills, and ability to practice independently while maintaining a collaborative relationship.
  • Comfortable utilizing electronic medical records and telehealth platforms.
  • Previous credentialing with Medicaid, Aetna, Anthem, Cigna, and UnitedHealthcare strongly preferred, but not required.
  • Willingness to assist with occasional medication management coverage for the other provider.

We welcome qualified FNPs with relevant psychiatric experience and are committed to ensuring providers practice within their education, training, experience, and applicable scope of practice.

  • W-2 employment with payroll taxes withheld.
  • Compensation based on actual collections, patient volume, and payer reimbursement.
  • Flexible scheduling and the ability to build a patient caseload over time.
  • Opportunity to increase earnings as patient volume grows.
  • Administrative support and insurance credentialing provided.
  • Remote work opportunities with optional in-person office space.
  • Employer-sponsored health insurance is not currently available but may be considered as the practice grows.
  • Applicable wage guarantees and compensation for required work will be addressed in the employment agreement in accordance with employment law.
