Tara Healthcare LLC is a telehealth practice built around medications that work when people can actually get to them. Our metabolic health program puts GLP-1 therapy in patients’ hands without the waitlists, the drive time, or the pharmacy runaround. We serve patients in 40 plus states, run on the Healthie EHR, and keep the administrative load off clinicians so they can spend their time on patients.

This role is our weight management line only. It is a non-controlled panel, so there is no DEA registration to maintain, no PDMP workflow, and no controlled substance registration burden. The volume is high and the visits are structured.

What You Will Do

• Conduct new patient evaluations and follow-up visits by video across the states where you are licensed, with asynchronous visits available when a patient requests that format
• Evaluate candidacy for GLP-1 therapy, including BMI and comorbidity criteria, contraindication screening (personal or family history of medullary thyroid carcinoma, MEN2, pancreatitis, gallbladder disease, active pregnancy), and medication reconciliation
• Prescribe and titrate tirzepatide and semaglutide, managing the full dose ladder from initiation through maintenance
• Manage side effects across the titration curve, including nausea, GI intolerance, dehydration, and dose-hold or dose-reduction decisions
• Counsel on nutrition, protein intake, muscle preservation, and realistic expectations for the pace of loss
• Order and interpret labs where clinically indicated
• Document visits in Healthie the same day, using our chart templates
• Respond to patient messages and refill requests inside our stated turnaround windows
• Coordinate with our pharmacy partners on prescription issues, clarifications, and shipping holds
• Escalate complex or high-risk cases to the Medical Director
• Help refine our titration protocols and standing orders as the program grows

Licensure Requirements

This is a high-coverage, multi-state role. The APRN Compact is not operational, and the Nurse Licensure Compact does not cover APRN practice, so we need clinicians who already hold individual active licenses across a wide footprint.

Required:

• Active, unrestricted NP (FNP, AGNP, or equivalent) or PA license in ten or more states
• National board certification (AANP, ANCC, or NCCPA) in good standing
• Clean license history, no board actions, no exclusions from federal healthcare programs
• Ability to pass credentialing and a background check

Not required for this role: DEA registration and state controlled substance registrations. This panel is entirely non-controlled.

Clinicians who maintain licensure in additional states, or who add states over time, will have more patient volume available to them.

Experience We Are Looking For

Required:

• Two plus years of independent prescribing experience as an NP or PA
• Direct experience prescribing and titrating GLP-1 therapy, including managing patients through GI side effects and dose holds
• Comfort practicing telehealth without an in-person safety net
• Strong, efficient documentation habits and same-day charting discipline
• The judgment to say no when a patient is not an appropriate candidate, and the communication skills to explain why

Nice to have:

• Background in obesity medicine, endocrinology, primary care, or bariatrics
• Obesity medicine certification or coursework
• Experience in a high-volume telehealth practice
• Familiarity with Healthie or a comparable modern EHR
• Experience with collaborative practice agreements in reduced or restricted practice states

Compensation

Per-visit contractor rate, paid monthly:

• Synchronous (video) visit: $35
• Asynchronous (message-based) visit: $15

GLP-1 patients follow a monthly titration and refill cadence, so a ten-state panel builds into predictable recurring volume rather than one-off visits.

As an independent contractor, you are responsible for your own state licensure, board certification, and continuing education. Malpractice coverage is provided through the practice.

We review applications weekly and typically move from first conversation to credentialing within two weeks.
